Athletic Physical - Required Forms

• Before you can participate in any physical activity with Belleview Athletics you must have an athletic physical form on file. The links below provide ALL necessary forms, please print and complete ALL forms and turn them into your coach. A doctor will need to complete the second page of the EL2 form, all other pages and sections need student and parent signatures.

There are two different physical packets for the 2026-27 school year (please read the descriptions below and choose the correct packet):

26-27 ECG Complete Physical Packet.pdf - this packet is for incoming freshmen, first-time athletes and anyone that is new to Florida school(s)

26-27 Complete Physical Packet.pdf - this packet is for returning athletes (10th, 11th, & 12th grades) and transfer athletes (10th, 11th, & 12th grades) within the state of Florida

NFHS Courses Required for All Student-Athletes

• IMPORTANT NOTE: THESE COURSES ARE REQUIRED ANNUALLY.

  The FHSAA requires all student-athletes for all sports at all levels to complete the free courses listed below prior to the first practice of their sport annually.

  • Sportsmanship
  • Sudden Cardiac Arrest
  • Heat Illness Prevention
